Chapter overview Management of the third stage of childbirth involves care practices regarding cutting of the umbilical cord and expulsion of the placenta and membranes. Yet, there is a growing amount of evidence to show routine intervention of immediate/early cord clamping practices in the third stage of childbirth are known to impact upon a range of important maternal and infant health outcomes, such as early onset infant anaemia. Several international organisations across the world are recommending delayed cord clamping (hereinafter known as optimal cord clamping) for all babies (NICE 2014; RCOG 2015; WHO 2014, ACOG 2017, ACNM 2014), This chapter provides the history of optimal cord clamping and focuses on the mid-wife's role and the decision-making within this practice.
